中国儿童及青少年中枢神经系统非生殖细胞瘤性生殖细胞肿瘤诊治指南(2026版)

摘要:

中枢神经系统生殖细胞肿瘤是常见于儿童和青少年的中枢神经系统恶性肿瘤,其中纯生殖细胞瘤对放疗和化疗的敏感性显著优于其他类型,故其他类型统称为非生殖细胞瘤性生殖细胞肿瘤(NGGCT)。中枢神经系统NGGCT的国际标准治疗采用手术、化疗与放疗相结合的综合治疗策略,但其亚型多样、病情复杂,治疗方案需结合患者年龄、临床症状、肿瘤标志物表达、肿瘤部位、大小及病理类型等因素个体化制定。为规范诊疗流程,中华医学会神经外科分会小儿神经外科学组基于国内外临床研究与实践经验,制定了中国儿童及青少年中枢神经系统NGGCT诊治指南。本指南涵盖3岁以上初发与复发患者的综合诊疗方案,旨在提升国内中枢神经系统NGGCT患者的治疗效果并改善预后。

Abstract:

Central nervous system germ cell tumors are common malignant tumors in children and adolescents. Pure germ cell tumors are significantly more sensitive to radiotherapy and chemotherapy than other types of germ cell tumors, so other types of germ cell tumors are collectively referred to as non-germinomatous germ cell tumors(NGGCT). The internationally recognized treatment method for central nervous system NGGCT is an integrated treatment approach combining surgery, chemotherapy, and radiotherapy. However, due to the diversity and complexity of its subtypes, treatment strategies also vary based on multiple related factors such as patient age, clinical symptoms, tumor marker expression, tumor location, tumor size, and pathological type. To standardize the diagnosis and treatment of central nervous system NGGCT, the Pediatric Neurosurgery Group of the Chinese Medical Association's Neurosurgery Branch has developed guidelines for the diagnosis and treatment of central nervous system NGGCT in Chinese children and adolescents based on domestic and international clinical research and experience. This guideline covers the integrated diagnosis and treatment plans for primary and recurrent central nervous system NGGCT in patients over 3 years old, hoping to further improve the treatment outcomes and prognosis of patients with central nervous system NGGCT in China.
